We will continuously attempt to keep all client information up to date and accurate.
We will regularly assess ourselves and our procedures to ensure that client information is properly safeguarded. Only
authorized Shepherd Village employees, who are trained in the proper handling of client information, will have access to
client information and any employee caught violating our Privacy Promise will be subject to disciplinary action. If other
organizations are contracted to assist in providing client care or services, we will require them to conform to our privacy
standards.
Upon request, any client may obtain a copy of our Privacy Policy by contacting our Chief Privacy Officer.
Any client wanting to review or verify their personal information may do so simply by contacting the appropriate
department Manager and making a formal request. You may then challenge the accuracy of the information and have it
amended as appropriate.
