The Shepherd Village Wellness Centre

• Medical care and service is available for the residents of Shepherd Village through the Wellness Centre
• To arrange for the transfer of your medical records and to book an appointment, please contact the Wellness Centre Nurse at 416-609-5700, extension 110

• Services include general physiotherapy, assessments for wheelchairs and walkers, occupational therapy referrals, exercises for preventive care for osteoarthritis and osteoporosis
• A written referral from your family doctor is required
• Fees are covered by OHIP
• Portable equipment: in special cases. in-home services (in your residence) may be provided by a registered physiotherapist
• The Wellness Centre nurse will submit the doctor's referral to the physiotherapist for follow up treatment

• An initial assessment is done at no charge.
• A written estimate of services is provided to residents or their family prior to proceeding with any work.
• Complete dental care is done on site at Shepherd Village. Golden Care Dental provides full dental services through the use of their portable dental equipment.
• The Wellness Centre Nurse at 416-609-5700, extension 100 may book appointments as required or appointments may be booked directly with Golden Care Dental at 416-484-6228.
• Fees for services are billed directly to residents as per the quotation from Golden Care Dental Services.

• Appointments are arranged by the Wellness Centre Nurse. Please call 416-609-5700, extension 110.
• Some restrictions apply under OHIP billing
• A written referral from your family doctor is required.
• Eyeglasses are also available from an optician who offers frames, fittings, repairs and adjustments. It will take approximately two weeks for eyeglasses to be prepared and delivered to you.

• Chiropodist services include anything involving soft tissue (i.e., corns, callus removal, nail cutting, orthodics, etc.)

• To arrange an appointment:

- Shepherd Gardens and Shephred Manor residents, contact the Wellness Centre Nurse, extension 110

- Shepherd Terrace and Shepherd Lodge residents, contact your Nurse

• Regular Clinics are offered for hearing, repairs and cleanings, hearing instrument consultations and ear impressions, hearing instrument adjustments and fittings.
• Residents and staff can arrange appointments by calling the hearing Instrument Practitioner, Scarborough Hearing Services at 416-298-4575.
• Full hearing assessments and evaluations can be done at the Wellness Centre. Ear, nose & throat consultations are available from a specialist.
• Fees for services are billed directly to you as per a quotation from Scarborough Hearing Services.

• Specializing in Cholesterol, Diabetes, Weight-loss, Digestive Disorders and General Nutrition
• Services are available weekly in the Wellness Centre, Wednesday afternoons
• Services are not covered by OHIP

• Services include Abdominal and Breast Scans, Doppler ultrasound including Carotid Arteries, Legs Veins and Leg Arteries
• All residents must have a doctor's referral
• Fees are covered by OHIP
• The Ultrasound clinic is available every second week or on emergency basis
• All referrals must be dropped off to the Wellness Centre Nurse prior to the appointment, or faxed.  Phone the Wellness Centre Nurse for more information at 416-609-5700, extension 110.

• Massage Therapy is available every second Friday from 12:00 - 5:00 pm
• Fee is $40.00 for each 30 minutes
• At each appointment resident signs an authorization form
• Fee for the service is added to your monthly bill

• Consultation for skin assessments available on monthly basis
• All residents must have a doctor's referral
• Fees are covered by OHIP
